Role of cardiac MRI in Ischaemic Heart Disease

Cardiac Magnetic Resonance (CMR) has an important role in the investigation and management of ischaemic heart disease by being able to answer accurately key questions.  These being ventricular function, viability and ischaemia?  Cine MRI is the gold standard for evaluating ventricular function and volumes, considered the gold standard for viability by using late gadolinium imaging to accurately assess the transmural extent of infarction and therefore able to predict functional recovery of dysfunctional myocardium in patients after MI. 

Stress perfusion MRI allows detection of subendocardial ischaemia with superior diagnostic accuracy.
CMR also has a role in the management of acute coronary syndrome,by being able to characterise the pathophysiology of acute myocardial infarction.  Risk stratification, reperfusion injury, prognosis, and also efficacy of treatment strategies can be assessed with CMR.

It is widely accepted that the CMR is an excellent tool and has a central role in the evaluation of IHD.

Consultant Cardio-thoracic Radiologist Essex Cardio-Thoracic Centre Dr Swamy Gedela is the Cardiac MRI (CMR) Director and the lead cardio-thoracic radiologist at the Essex CTC. He developed and pioneered the creation of the Essex CTC CMR Unit which he also leads providing cardiac MRI for over 5 referring NHS trusts. It is going through rapid expansion, accelerated to already performing approx. 1700 cardiac MRI examinations per year and expected to perform 2000-3000/year in the near future. He leads the cardiac CT service, continuing to adapt and modify protocols and performs complex cardiac CT imaging at a high level. He also provides expert second opinions on cardio-thoracic CT and MRI imaging. He is committed and passionate in enhancing and developing the clinical use of cardiac MRI and cardiac CT in the management of patients with Heart Disease. His areas of expertise includes Cardiac MRI, Coronary CT angiography, Coronary Calcium scoring, Thoracic oncology CT and High Resolution thoracic CT.
